How Do I Request Corrections to INS or USCIS Documents Due to INS or USCIS Administrative Error?

Permanent Resident Card (I-551)
To correct a Permanent Resident Card (I-551), you must file an
form I-90. IF the error was the fault of INS or USCIS, you do NOT have to pay the fee. IF the error was your fault, you DO need to pay the fee.

Note that the bottom of page 1 of the Instructions to the I-90 says:
"A replacement application based on administrative INS or USCIS error must also include an explanation."

Therefore, please attach to your I-90 your incorrect Permanent Resident Card (greencard) and, if possible, a photocopy of the original application that you submitted that had the correct information on it (as proof that your original documentation was correct).

On the I-90 form itself, under "Part 2: Application Type," you should mark "2.d." as the reason for filing (NOTE: "2.d." states: "My card was issued with incorrect information because of INS administrative error. I have attached the incorrect card and evidence of the correct information.")

Again, you do NOT have to pay any fee if the error was made by INS or USCIS.

Naturalization Certificate
To correct a Naturalization Certificate, IF a clerical error was made, file a Form N-565 without fee at the USCIS office with jurisdiction over your residence. IF the error was your fault, the fee must be paid

Refugee Travel Document, Reentry Permit, or Certificate of Citizenship
To correct a Refugee Travel Document, Reentry Permit, or Certificate of Citizenship, you need to submit a letter along with the incorrect document. No form or fee is required.

I-94, Arrival-Departure Record
To correct an I-94, Arrival-Departure Record (obtained at the port of entry during admission), file Form I-102. IF the error was the fault of the border inspector or USCIS, you do NOT have to pay the fee. If the error was your fault, you DO need to pay the fee.

Address Change
All non-U.S. citizens (aliens) who are required to be registered are also required to keep the USCIS informed of their current address. For information about this, see How Do I Report a Change of Address to the USCIS.
